External Domain User Account Registration Form

This form is to request access for non-APL employees to a SharePoint site.

The requestor agrees that the user account being requested will be used solely by the requestor. Data and resources accessed by this account will be used as directed by the sponsoring Johns Hopkins University Applied Physics Laboratory ("APL") program as set forth at the specific service for which access is being requested.

Furthermore, APL reserves the right, in its sole discretion, to change the terms, conditions, and notices under which the requestor may access a service and/or to terminate requestor's access to any and all resources granted by APL including automatic termination of access if requestor's account remains inactive for more than 90 days.
